Grey coloured card cover with four page insert of cream paper, printed with “Ship / Engaged, Discharged / Signature of Master / or Official Stamp”. All details are printed in black ink with details handwritten in black ink. Booklet number printed in top right corner, “No. 103713”. Inside front cover is a black and white passport photograph of Raymond WARBURTON in bottom left corner, and with a blue stamp of, “M.M. OFFICE 12 OCT 1918 AVONMOUTH”, stamped across photograph.

Front cover, under headings of, “Date and Place of Issue” is handwritten details – “7.10.18 Avonmouth”. Identity details on inside cover, under headings, “Date of Birth / Place of Birth”, “28 July 1891 / Sydney Australia”. One entry appears on first insert page, “Ship Shropshire / Engaged 21.10.18”. Back cover under heading, “No. of Discharge Book (if any)”, is handwritten detail, “882058”. Signature of “R Warburton”, appears in middle of back cover.
With the outbreak of the First World War Raymond Parker WARBURTON was employed as an Electrical Engineer aboard troop transport ships. Raymond took up service between 01/12/1915 and 09/09/1917, first serving on the HMAT Pera and then from 19/12/1917 to 13/06/1919 he served on the ‘SS Shropshire’. This Identity Card shows the entry of his service on the SS Shropshire, dated 21.10.18. And on the back cover lists his next of kin, his mother Annie WARBURTON, address 109 Kent St SYDNEY.
