Typed in black ink on yellow paper with embossed Royal Coat of Arms in top left-hand corner. Signed by hand in ink by the Acting Secretary. Very minor foxing.

“Home Office / Whitehall / S.W. 1 / 28th January 1920 / CONFIDENTIAL / ORDER OF THE BRITISH EMPIRE / Sir / I am directed by the Home Secretary, on behalf of / the Prime Minister, to inform you that, in view of the / service you have rendered on work connected with the War / it is proposed to submit your name to the King for appointment / as a Member of the Order of the British Empire (Civil Division) / The Home Secretary desires me to ask you to be so / good as to fill up the enclosed form and return it to me / in the accompanying addressed envelope at your earliest / convenience. / I am, / Sir, / Your obedient Servant, / G.G.
