JOHN CUSACK HOLE IN ONE TROPHY 2000
2000John Cusack achieved his third hole in one in 2000. Recognition of the hole in one consisted of five unique items during that period. Each of those items is described below.
Part a - Trophy
The trophy consists of a plastic number one connected to a tee which together hold the ball John used for that feat. This is mounted on a wood grain laminated MDF (wooden) base which bears a plaque with the inscription:
‘MELVILLE GLADES GOLF CLUB
17TH HOLE (122M)
11th OCTOBER 2000
JOHN CUSACK’
The ball used by John was a Spalding HOT DOT, numbered 2 in red.
Part b – WAGA Certificate
The Western Australian Golf Association (WAGA) provided this certificate. The A5 sized certificate is printed on an ivory-coloured piece of light cardboard and signed by the Executive Director of the WAGA, Greg Fitzhardinge.
Part c – Johnnie Walker Award
For a certain period, holes in one were recognised by Johnnie Walker Whisky. This certificate is one of those. The award was accompanied by a complimentary bottle of whisky. (The whisky bottle was not available!)
Part d – The West Australian Certificate
This buff-coloured light cardboard certificate has a tartan border frame on a golfer patterned background with a “Congratulations” statement and was provided by The West Australian newspaper. It welcomes the person into their Hole in One Club and is complementary to the medallion described below.
Part e – The West Australian Medallion
This a brass medallion fitted with a lanyard, used to attach the medallion to a golf bag. The West Australian newspaper has sponsored this medallion.
On the front are the words:
‘The West Australian (in what is believed to be Gothic Medieval font)

On the back are the words:
