A certificate awarded to Charles W. Levitzke of the Katnning Railway Ambulance Division, Western Australia District, Commonwealth of Australia stating that he is able to discharge the duties of an Ambulance Officer. In the top left hand corner is a white Maltese cross on a black background, which is the logo for St John Ambulance.
