A leather covered book with parchment pages, enclosed in a deteriorating leather cover.

The inscription on the cover reads 'Honour Roll St Andrews Subiaco 1914'.

An inscription inside reads 'This Honour Roll was dedicated by His Grace the Lord Archbishop of Perth, Sunday August 26th 1917. R.J. Craggs, Rector; W.O. Gregson and C.W. Mudie, Church Wardens; cover donated by W.H. Henderson; roll inscribed by F.J. Crockett'. There is a second cover of embossed navy leather, with a coat of arms on the back. Inside are 24 parchment pages, all with gold paint surround making ornate crosses in each corner. The frontispiece reads: 'St Andrews, Subiaco, Western Australia Roll of Honour - in memory of those members of the Church who volunteered for God King and Country in the Great War 1914-1919. Rev. R .J. Craggs, Rector. W.O. Gregson, C.W. Mudie, Church Wardens'.

Each page has a red letter of the alphabet with the names of soldiers from the Parish of St Andrews inscribed in alphabetical order. Some names have red asterisks (probably indicating the serviceman was killed in action). Attached to the top of the badly damaged spine of this first cover is the remnants of a piece of wire for attaching a chain, as the book was originally chained to a purpose built wooden stand. The front of the cover has two studs, and the back has a flap of leather which would have held fasteners to the studs.
